Appointment scheduling for active patients who need follow-up appointments

  • We are maintaining a list of all active patients who are due for follow-up after the closure of Optum/CCAO on May 20,2026.
  • WE WILL REACH OUT TO YOU in the coming weeks to schedule your follow-up with Dr. Perkins at his new practice after July 13.
  • If you have not received a communication to schedule your follow-up appointment by May 20, please give Dr. Perkins' new office a call and schedule an appointment. The phone number for his BCH Rheumatology Associates office after May 20 is (303) 415-7879.
  • Please be patient as we work through all our patients who are due for follow-up. we will get to you as soon as we can.

Patients receiving in-office infusions and injections

  • For patients who are on a medication that we administer in our office, our BCH practice is working to provide services to our patients as soon as possible.
  • There will be an interim period where Dr. Perkins may need to refer you to an outside provider for continuation of your medication.
  • Our infusion patients who are due for medication in the first weeks after the Optum/CCAO closure will receive our top priority. We will be reaching out to each individual patient to coordinate uninterrupted scheduling of these vital medications in the coming weeks.
  • If you are an active infusion patient and have NOT received communication from Dr. Perkins' current practice by May 1, 2026, please call us at 720-494-4700 or message us through your MyChart account.

Scheduling a follow-up appointment on your own

Dr. Perkins's schedule template at BCH is expected to be active beginning May 20, 2026. You may call (303) 415-7879 after this date to schedule an appointment with Dr. Perkins.

What if I have a medical need prior to July 13?

  • Prior to May 20, 2026, call (720) 494-4700 or send a MyChart message to Dr. Perkins as his current practice, as usual
  • As of May 20, 2026, call Dr. Perkins' new practice at (303) 415-7879.
  • 24/7 on-call coverage for urgent medical issues will continue to be available.

Longmont Location

  • INTERIM LOCATION (7/13/26 through early 2027): While the BCH Longmont office is being expanded, Dr. Perkins will be seeing his Longmont patients at an interim site at 6685 Gunpark Dr., Suite 110, in the Gunbarrel area. This is approximately 7 miles west on Hwy 119/Diagonal Hwy.
  • PERMANENT LOCATION (January 2027): Dr. Perkins will begin seeing patients at the BCH Longmont site at 2101 Ken Pratt Blvd. beginning in January 2027.
  • He will see patients Tuesday through Friday at the above locations.

Superior Location

  • Beginning July 17, 2026, Dr. Perkins will begin seeing patients at the BCH Superior office, located at 3 Superior Dr, Suite 100B.
  • He will be in the Superior location on alternate Fridays.
